Leeds Outreach may can help you with utilities, prescription medicine or gasoline to get to the doctor.
Leeds Outreach asks for truthful answers to all interview questions. If it is determined you have given information that is untrue or misleading, you will no longer be eligible for assistance. Be advised LO uses Charity Tracker which allows us to view assistance you may receive from similar agencies. LO has a lifetime maximum of $1500 for all financial assistance.
Proofs required for assistance:
- Drivers License, Birth Certificate or Photo ID
Lease - Utility Bill (city limits of Leeds only)
- Proof of ALL income for the entire household including, but not limited to: SSA, SSI, Wages, Child Support, TANF, Food Stamps
- If unemployed for over 3 months you must provide some proof of how you are paying your bills. Accepted proofs include: prior year tax return, letter, with phone number, from anyone that offers financial help (this will be verified).
